Здравствуйте.

Документация клинического исследования. Вопрос: как лучше всего перевести термин exposure в этом контексте. Препарат вводили внутрь, измеряли концентрацию в крови. Правильно ли я понимаю, что под peak exposure ниже имеется в виду максимальная концентрация препарата в крови? И тогда total exposure - должно бы быть area under curve? Или не так? Был бы препарат местного действия - писал бы экспозиция, но он системный, поэтому не знаю как быть. Хотя я нашел статью в интернете, где пишут "экспозиция" ( http://www.nephro.ru/magazine/abstract.php?id=14963 ) .

Exposure measures the interaction/contact of an organism and a compound over a specified time period. The area under the curve (AUC) is an important parameter for exposure. It is a measure of how much of a drug reaches the bloodstream in a set period of time. AUC is calculated by plotting drug blood concentration at various times during a 24-hour or longer period and then measuring the AUC between 0 and 24 hours.

Ниже мой контекст:

Peak and total exposures increased in a linear manner across 10 to 20 mg doses on Days 1 and 12. No accumulation resulted from QD dosing at 15 or 20 mg. Measurable accumulation was seen during BID dosing, with steady state achieved within appr. 2 days.

The total number of days of treatment, total number of doses, average daily dose, total exposure (total amount [mg or IU]), and total exposure per kg will be summarized for each treatment group

The Center for Drug Evaluation and Research (CDER), FDA, is aware of the shortcomings of parameters like AUC, and for the evaluation of the equivalence of two medicinal products:

"Both direct (e. g., rate constant, rate profile) and indirect (e. g., Cmax, Tmax, mean absorption time, mean residence time, Cmax normalized to AUC) pharmacokinetic measures are limited in their ability to assess rate of absorption. This guidance, therefore, recommends a change in focus from these direct or indirect measures of absorption rate to measures of systemic exposure."

Surprisingly, although the CDER introduces three new parameters, namely Early Exposure, measured by AUC from 0 to Tmax, Peak Exposure, measured by Cmax and Total Exposure, measured by AUC from 0 to ∞, these three “new” parameters are new only in name, and are not much different from the old parameters AUC, Cmax and Tmax, whose shortcomings are well recognized.

Добрый день!
Не смотря на приведенную цитату не смогла разобраться как корректно перевести "peak (Cmax), early (based on AUC0-8), and overall (based on AUClast) systemic exposure" ...
Очень надеюсь на любую помощь!
